Change of Address

  • District Office Student Assignment Summer Hours: Monday through Thursday from 8:00 a.m. to 4:00 p.m.

    Registration (including change of address) Hours: Monday through Thursday from 8:30 a.m. to 2:30 p.m.

    Student Assignment will be closed on June 21, June 28, 2019

    Student Assignment will be closed on July 4, July 5, July 12, July 19, July 26, 2019

    Student Assignment will be closed on August 2, August 13, August 14, 2019

     

    Any change in residence must be reported to the Student Assignment Office located at 1309 S. Mary Avenue Suite #150, Sunnyvale, CA 94087 within ten (10) working days.  Please bring the Proof of Residence information listed below:

     

    Proof of Residence – One (1) original document is required (copy will be made at the time of registration)

    • Homeowner:
      • Current Original Property Tax Bill – with parent/guardian’s name, mailing address and property address.  One may obtain a duplicate of his/her original property tax bill at the

                                    Department of Tax and Collections

                                    70 W. Hedding Street, East Wing, 6th floor

                                    San Jose, 95110

      • Final Buyer Statement (if purchased within last six [6] months) – with parent/guardian’s name and property address
    • CUSD will not accept a Grant Deed as proof of residence.
    • CUSD will not accept a Property Assessment mailed by the Santa Clara County Assessors Office.
    • Renter/Lessee:
      • Printed fully executed lease agreement – includes signatures from both parent/guardian and manager/landlord and Manager/Landlord’s contact information
    • Affidavit of residence (family living with another resident):
      • An Affidavit of Parent Residence form must be signed by parent and the primary resident in the Student Assignment Office
      • Primary resident must provide Property Tax Bill or fully executed lease agreement and photo identification
      • Parent must provide two additional forms of identification with current address on it (most recent bank statement, car insurance or registration, or paycheck stub) 

     

    Moving Within the District

    Grades Kindergarten - grade 4 and grade 6 - grade 7
    Students currently attending a District school in grades Kindergarten through 4 and grades 6 and 7 who have a change of residence out of the school attendance area, but within the District, must provide current proof of residence and complete a Request For Transfer form available at the Student Assignment Office. Transfer requests will be considered on an individual basis at the time of the request.  

    Note: Transfer requests will not be granted if neighborhood students are being enrolled to an overflow school.

    Grade 5 and grade 8
    Students currently attending a District school in grades 5 and 8 who move out of the school attendance area, but reside within the District, will be allowed to complete the school year pending space availability. The parent will need to provide current proof of residence and completes a Request For Transfer form available at the Student Assignment Office.

    Note: Transfer requests will not be granted if neighborhood students are being enrolled to an overflow school.

    Moving Out of the District 

    Grades Kindergarten - grade 4 and grade 6 - grade 7
    Students currently attending a District school in grades Kindergarten through 4 and grades 6 and 7 who move out of the District will be processed using the following guidelines:

    • To allow a student to complete a school year when his/her parent/guardian has moved out of the district, the following conditions will be required:
      1. Adequate space is available at the current school 
      2. Parent completes a Request For Transfer form, available at the Student Assignment Office, requesting his/her child continue at the current school 
      3. Parent applies and receives an approved interdistrict transfer from his/her district of residence 
      4. The student adheres to the provisions of the transfer

    Note: Transfer requests will not be granted if there is a waitlist of In-District students. Transfers will be considered on an individual basis at the time of the request.

    Grade 5 and grade 8
    Students currently attending a District school in grades 5 and 8 who move out of the District will be allowed to complete the school year if all of the following conditions are met:

    1. Adequate space is available at the current school 
    2. Parent completes a Request For Transfer form, available at the Student Assignment Office, requesting his/her child continue at the current school 
    3. Parent applies and receives an approved interdistrict transfer from his/her district of residence 
    4. The student adheres to the provisions of the transfer

    Note: Transfer requests will not be granted if there is a waitlist of In-District students. Transfers will be considered on an individual basis at the time of the request.
