Change of Address

Did You Move? Update Your Address

If your family has moved, you must submit a Change of Address (COA) Request within 10 days of your move date.

Step 1: Submit the request through the Change of Address request form.

Step 2: Provide Documentation

  • To complete your request, you must provide full proof of residency when contacted by Student Assignment staff.
    • Proof of residence - Property Tax Bill or fully executed lease agreement. If you are living with another person, please let the staff member know who contacts you so you can request an affidavit of parent residence.
    • Additional proofs of residence - if you just moved into your CUSD address, staff will give 30 days to submit these documents
    • PG&E bill from last 30 days
    • One additional form - car registration, car insurance, paycheck stub or bank statement
    • Parent photo ID - state-issued ID or driver’s license or passport

If your move changes your home attendance area school, you have two options:

  • Immediate Transfer: Request to move to your new neighborhood school immediately (pending space availability).
  • Request to Remain: Request to stay at your current school site for the remainder of the current school year only. Important Note: Permission to remain at your current school is contingent upon space availability and overflow capacity. If granted, this placement is valid for the current school year only, and you will transition to your new attendance area school the following year.
